Available Shifts:

Monday-Friday 8:30am-5:00pm

Expected Hours of Work: Much of the work will be performed in a combination of the company's office and in the field during work hours. This position is expected to meet the needs of the business.

Qualifications

  • Professional Experience/Education/Certifications: Minimum High School graduate or GED equivalent required
  • Bachelor’s degree in Psychology, Behavioral Science or any related discipline required and/or a minimum of two (2) years of full-time experience with Individuals with dual diagnosis or forensic involvement required.
  • Practical experience in the forensic (legal) arena with clients and their families preferred
  • Certified in Non-violent Crisis Prevention required or must successfully complete in-house training
  • Must have a current CPR and First Aid card or must successfully complete in-house training
  • Must have a Direct Support Professional (DSP) I and II Certification or must successfully complete state-provided training for DSP I within six (6) months of hire and DSP II within .twelve (12) months of hire
  • Completion of a minimum of 12 hours of continuing education units annually required
  • Ability to speak, read, and write in English is required. The ability to speak Spanish is highly desired.

Primary Responsibilities:

The focus of a Community Support Specialist is to work directly with the Individuals, their families, and their circles of support. This position encompasses generic resources that allows for Individuals to enhance their services and life in decision-making with the guidance through coordinated life services. This position provides support to Individuals with developmental disabilities, through person-centered planning.

A Community Support Specialist assists in building community partnerships that would enhance resources for Individuals to utilize in their community. This position is responsible for intensive case coordination through partnerships with Individuals, families, and other agencies and is also responsible for advocating for the Individuals in court proceedings as part of the Judicial Intervention and Diversion Compliance Program.

Essential Functions:

  • Coordinates individuals’ and their family members’ access to all necessary community, physical and mental health services.
  • Analyzes community trends to determine available resources in the market.
  • Provides a person-centered approach to support all assigned Individuals.
  • Builds and maintains professional relationships and partnerships with outside agencies (including the regional centers) and the Individuals and their families.
  • Maintains open lines of communication with Individuals, administrators, parents, and conservators.
  • Initiates and completes intake assessment which includes interviewing the Individual and their family member to determine services needed.
  • Completes daily case notes and quarterly progress reports based on Individuals’ progress towards goals identified in the ISP and IPP.
  • Initiates and completes Special Incident Reports (SIRs) for submission to the regional centers.
  • Oversees service hours for assigned Individuals to ensure they are being provided and accurately recorded for monthly submission.
  • Coordinates and advocates the Individuals’ appointments, court dates, inter-agency involvement, volunteer hours, work schedules, etc. to promote their attendance and participation.
  • Provides transportation to Individuals to attend outside appointments.
  • Participates in meetings with families, schools and outside agencies regarding the Individuals’ progress or support services.
  • Establishes and maintains the highest level of quality service to all Individuals and stakeholders.
  • Ensures non-discrimination in the delivery of program services and assistance.
  • Attends all required trainings and staff meetings essential for the position.
  • Performs all other duties and responsibilities, as assigned.
