  • CA Driver’s License
  • Caregiving: 1 year (Preferred)

Primary Responsibilities:

;

The goal of our Independent Living Services (ILS) Instructors it to assist our clients who are adults with intellectual and developmental disabilities and wish to live in their own homes.;Our caring Instructors will assist these individuals create a personalized home environment, set their own routines and work towards creating a safe home and community experience. We encourage and support building strong partnerships with family, friends, neighbors, and other community members.;

  • ;Works with Individuals with severe behavioral challenges to meet their desired outcomes.
  • Provides day and/or night close monitoring for either behaviorally challenged and/or medically fragile Individuals.
  • ;Ensures and monitors that the Individuals self-administer their medications correctly as prescribed.
  • Works directly with Individuals to provide support in remediation of severe behaviors that are endangering current placement.
  • Establishes a trusting and caring relationship with the Individuals and those working with them.
  • Works cooperatively with the Individuals, authorized representatives, and members of the participants’ circle of support.
  • Promotes a safe, clean, and pleasant living environment.
  • Reports any incidents as required of a Mandated Reporter immediately and reports special incidents as they occur.
  • Ensures that all required documentation is completed accurately and timely.
  • Familiarize themselves with the participants’ Person-Centered Plans (IPPs) and Individual Service Plans (ISPs) and goals.
  • Provides training and habilitation for the Individuals to fulfill the health and safety requirements which are specific to the needs of the Individuals to include:

1.;;;;Activities for daily living (ADLs)

2.;;;;Mobility training

3.;;;;Developing coping skills

4.;;;;Navigating the community

5.;;;;Safe self-medication management

6.;;;;Money management

7.;;;;Other areas as indicated on the ISP

  • Monitors physical needs and/or identifies any triggers of the Individuals and reports any changes in desired outcomes.
  • Provides person-centered and accurate record-keeping and data systems daily to provide specific instruction and/or supports.
  • Utilizes person-centered communications skills to help the participants to identify what is important to and for them.
  • Adjusts to daily schedule changes or emergencies in a safe and effective manner.
  • Ensures that personal care, including transferring and repositioning, are safely completed for Individuals who have limited mobility.
  • When appropriate, collaborates with family members and/or members of the circle of support to ensure effective communication regarding the Individuals.
  • Communicates effectively through verbal and written skills with different agencies and the public.
  • Familiarizes oneself with the resources available within the communities to support the Individuals’ needs and preferences.
  • Advocates for the Individuals’ Rights while reminding them of their Responsibilities.
  • Attends staff meetings and in-service training, as required.
  • Complies with all company policies, procedures, rules, and regulations.
  • Performs other duties and responsibilities, as assigned.
